In the new Surviving Sepsis Guidance, a revised "hour one care bundle" has been published. Key features include:

-Measure lactate and repeat if > 2 mmol/L

-Obtain blood cultures

-Administer broad spectrum antibiotics

-Rapid administration of 30mls/kg of crystalloid for hypotension or lactate > 4 mmol/L

-Apply vasopressors if hypotensive during or after fluid resuscitation to maintain MAP > 65mmHg

This replaces the pre-existing 3 and 6 hour bundles.
